A skills and health assessment is a professional and individual assessment of the needs of individuals for permanent residence in nursing and residential homes. The assessment is standardized together with defined information from health and social services and the regional office for the disabled as well as medical letters from a doctor as appropriate. A skills and health assessment is also carried out for a temporary rest stay in a nursing home.

Rest and rehabilitation admissions to the hospital in Ísafjörður are outside this system, and are applied for through a doctor.

A special committee operates for each health district. Elsa Magnúsdóttir (elsa.magnusdottir@hvest.is), a home care nurse in Ísafjörður, is the committee's contact person in Vestfjörður.

The committee includes:

  • Elsa Magnúsdóttir, nurse at the Vestfjörður Health Center, chairperson
  • Margrét Geirsdóttir welfare department of Ísafjarðarbær
  • Jón Eyjólfur Jónsson, geriatrician at Landspítala

As a general rule, it is assumed that no more than 5-6 weeks will pass from the time the committee receives a request for a skill and health assessment or a rest period until the result is known.

The result of the committee's assessment is announced in writing when it is available.

Please note that the address of the institution is written incorrectly at the bottom of the application form, it is best to send it to Elsa Magnúsdóttir at the Vestfjörður Health Institute, Torfnesi, 400 Ísafjörður.
