The Vestfj\u00f6r\u00f0ur Health Institute and Vesturbygg\u00f0 have reached an agreement on the operation of integrated home care in the municipality. The contract is part of the project\u00a0Good to grow old<\/em>, the government's action plan for older people<\/a><\/a><\/p>\n\n\n\n

The Institute of Health will undertake the operation of home support in collaboration with the municipality. Emphasis will be placed on a person-centered approach where professionals work together in teamwork. There will be a single service portal, which means that all requests for services will go through a single system and their assessment will be a joint task of social and health professionals.<\/p>\n\n\n\n

The goal of the project is to increase the quality of services for older people, reduce the likelihood that people will fall between service systems and reduce the number of admissions to hospitals and nursing homes. With the integration of social and healthcare services, the aim is for the services to be in one flow and to respond quickly to the changing needs of users.<\/p>\n\n\n\n

With this agreement, an important step has been taken towards better and more efficient services for older people in Vesturbygg\u00f0, where the maximum use of funds is guaranteed without compromising the quality of the service. It is hoped that this integrated home service will lead to a better quality of life for the municipality's elderly residents.<\/p>\n\n\n\n
